All footage or images are to be used by Puddle Ducks Franchising Limited for the purposes of promoting our lessons to potential new and existing customers.

I understand that the videos/images will not be used for resale purposes by Puddle Ducks; and

Puddle Ducks will only use the videos/images for the purposes of promoting Puddle Ducks lessons on our website, on social media, in digital marketing, in email campaigns and in promotional print material.

I confirm that I am the parent or legal guardian of the child in the video and therefore have the authority to sign below to consent to the use of the video as described in this form.
