Nomination Form

2025 sees Puddle Ducks Lincolnshire turn 15 years old! As part of our birthday celebrations, we want to give back to the community who has supported us.

We will be giving 15 babies or children the opportunity to swim ABSOLUTELY FREE! The lessons will start after the May half term break and run through until the summer holidays.

Do you know someone deserving of this amazing opportunity that you could nominate? Nominations can be submitted until Sunday 25th May and must be submitted using the form below.

Nominees must fulfil the following criteria:

* Must not already be swimming with us or must not have swam with us in the past

* Must be aged 0- 10 years

* Must be able to start swimming w/c 2nd June 2025

The reason(s) for the nomination must be fully explained on the nomination form in no more than 150 words and could include, for example, a bereavement, suffering from a serious health condition, a child who has been through a traumatic time or a child from a family struggling with the cost of living.

We will be inviting an independent judge to choose the lucky recipients of this amazing prize.

CLOSING DATE FOR NOMINATIONS: SUNDAY 25TH MAY 2025

T&Cs

* The free lessons start w/c 2nd June 2025 and are available during June and July 2025 ONLY. No replacement lessons or alternative start date will be offered if the recipients are unable to start swimming at this time.

* Courtesy classes may be booked for missed lessons via the online My Puddle Ducks account

* Free lessons can only be booked in classes where we have availability from w/c 2nd June 2025.
